The famous painting Pala di Brera was executed in 1472-1474 by the italian renaissance master Piero della Francesca.

Different steps, different links of the same chain
A chain is only as strong as its weakest link
It is mandatory to construct an optimal procedure from start to finish, because a chain is only as strong as its weakest link.
If one the link of the chain is weak, then all the procedure will be compromised.
In the case of the Piero’s chain, the egg will fall down. In the case of fat transfer, the fat cells will not survive and will not regenerate in the recipient area.
Fat harvesting is the procedure of obtaining adipose tissue through the use of suction cannulas.
The patented Goisis micro-cannula has a small diameter (2mm) with blunt tip and 6 ports.
The main advantages of the Goisis cannula are related to faster times of treatment compared with others Microcannulas. In fact, Aspiration of fat with Goisis Cannula is performed with a rotational movement.
The depressed edge promotes the entrance of fat into the hole.
The raised edge facilitates the dissection of the fat particles.
In this way, the shape of the multiple ports yields a big volume of aspirated tissue with each rotational stroke. The use of Goisis cannula is less time-consuming than Coleman harvesting cannula (approximately 2 times faster compared with Coleman 3mm cannula). The microcannula reduces patient trauma and discomfort; it reduces the risk of skin irregularities; and it minimizes the risk of vascular injury due to multiple reduced-size ports. Because the insertion requires less force and is more precise, the microcannula allows for more precise control, more superficial removal of fat (reducing the risk of accidental injuries of deeper tissues), and less scars due to the microincisions required.
Histological examinations performed on 247 samples demonstrated that harvested with Goisis cannula is associated with a better preservation of fat.

The first aim of the Graft processing is to remove contaminants and water, avoiding the contact with air. In fact, contaminants as blood cells, local anesthesia and oil have a pro inflammatory and toxic effect on fat. Despite of that, water mixed with fat creates false volume. Also the direct contact with air has a negative effect, because of activation of oxidative damages.
The innovation of Goisis method is that is a completely closed system. The harvest, the processing and the injection of the fat are performed into the system, avoiding the contact with air.
Processing by decantation
Simple decantation allows a significantly greater amount of aqueous and contaminants to remain in the specimen. Particularly red blood cells (red arrows) and free oil (blu arrows), that are pro-inflammatory, and local anesthesia, which is toxic. Recent scientific evidences confirm this, indicating lower rates of decanted graft viability compared to centrifuged and washed specimens.
Standard centrifugation led to high percentage of adipocyte death, with an high percentage of oil cysts (blu arrow).
The integrity and the viability of the adipocytes and the removal of the nonviable, proinflammatory and toxic components is guarantee by the washing and filtering Goisis system.
Classical liposuction harvesting cannulas have large port size increases. The harvested fat has an high viscosity, so if it is pushed through a small cannula at the moment of injection will produce a high shear stress. This shear stress will result in irreversible damage, and the mechanical integrity of the fat cannot fully recover from this damage.
Goisis system has small harvesting cannula, and the microfat has a very low viscosity. In this way, the injection is performed with small 22 Gauge cannula.
The use of small cannula creates less aedema, pain and brusing. The dead space interfering in cell integration is reduced, and it’s very simple to inject fat into the muscular plane.
Simplicity and efficiency
The Goisis System is very simple, because is based on mono-directional check valves. These valves open automatically: the fat and the blood are directed and moved into the different part of the system without need of turn valves.
The efficiency of lipofilling method can be valuated in term of time consuming and in term of loosing of fat during the preparation.
In term of time consuming, usually, the use of small cannulas with small ports reduces the viscosity of the harvested fat but increases the time of the procedure. An exception is the Goisis cannula. In fact, the shape of the multiple ports yields a big volume of aspirated tissue with each rotational stroke. The low viscosity of fat guarantees faster and simpler injection with small cannula.
Some low-efficiency methods requires the harvest of 100cc or more of fat to obtain 20cc of processed fat. The others 80 cc are loose into the processing system. With the Goisis system there is a dispersion of 3cc of fat. This means that 23cc of fat are sufficient to have 20cc of processed fat ready to be injected
